My experience in these settings is wider than in hospitals because patients can receive adequate care while at home supported by the family members. Home health and community care improve patient outcomes while providing care in patient-preferred setting. This form of care ensures patients, particularly the elderly, receive high-quality care in the comfort of their homes. Delivering health care at home or community setting increases my ability to provide patient-centered care by developing an individualized care plan. Furthermore, information technology greatly influences home health and community-based system. The use of information systems in these types of services helps patients to share information in real time with the health care teams. Moreover, this form of care increases the patient’s ability to manage their health and become independent.
I am expecting to interact more with the home health and related community-based systems due to the increasing need of safe transitions to improve clinical outcomes. The demand for home-based care will continue to increase to ensure that healthcare providers meet patients’ needs in the evolving US health care system. This field of nursing is changing rapidly and becoming an essential component in a healthcare system due to aging population and technological advancement which increases the ability to better care for patients outside the hospital setting. Technological advancement including those used to monitor patient behaviors holds a great promise in improving patient care either in home or community setting.
Health conditions that required hospitalization are now managed at home. The utilization of home health and the community-based related system is likely to continue increasing with the increase in chronic conditions and functional impairment associated with aging.
