You don't have access to the lounges. If you're one of the airline's new customers and don't have "elite status," your access to the lounges is likely to be limited. The last chance to shower and rest before your trip has passed.
There is no airport transfer or limousine pickup, so if you rely on airport transfers for international flights, Basic Business Class isn't for you. The majority of airlines do not charge extra for this service.
You don't have the option to pre-book a ticket until 48 hours before departure – if you choose Basic Business Class, you risk the plane's availability of seats, which might be inconvenient, especially if you're used to the normal Business Class's array of amenities.
Some airlines that provide Basic Business Class do not enable you to change or cancel your flight. If you have this opportunity, you will also incur significantly higher costs.
Mileage accrual is reduced for frequent flyers.
